I would like to please request the following information with regards to your Tier 2 weight management service, if applicable:
How many patients do you currently have on your waiting list?
How many patients have you referred in the last 12 months?
Is the service you offer commissioned in collaboration with the local CCG?
If you do not have a tier 2 weight management service, then please reply saying so.

I would like to please request the following information with regards to your Tier 2 weight management service, if applicable:
Q1 How many patients do you currently have on your waiting list?
The programme is very new, having only started in October. There is no waiting list as the programmes currently running are closed groups. We are in the process of generating interest in our new groups to commence in January.
Q2 How many patients have you referred in the last 12 months?
We have not referred any to date. >10 have been self -referrals and <5 have been referred by health professionals. Mums who come to the programme through the Health Trainer service are treated as self-referrals.
Q3 Is the service you offer commissioned in collaboration with the local CCG?
No, it's a council initiative.
Q4 If you do not have a tier 2 weight management service, then please reply saying so.
