I am making a Freedom of Information Act request for the following:

A copy of your referral form for Supported Living

Herefordshire Council do not have a referral form for supported living as we don't provide this service ourselves. Each supported living scheme has a different referral process for both the housing and support provider.

If a person during the course of a strength based assessment or review under The Care Act is assessed as needing supported living the worker may fill in an accommodation request on Mosaic and then pass to the Accommodation Team who will Triage and make recommendations based on presenting needs and local availability, alongside this support can be sort through the provider portals that are supported by the Brokerage Team.

Please see above - the only refusal the Local Authority would make is based on meeting eligible needs under The Care Act.

A copy of the National guidance

This would be Care Act guidance for the Local Authority, there is no national guidance on supported living although most support providers are CQC registered and most housing providers are registered social landlords and would fall under the auspices of the Housing Ombudsman.
