1. Does the local authority currently use Supported Living, 16+ services?
2. If so, how many are currently registered within your borough?
3. Is there a recommended service provider for Supported Living list?
4. How would a new provider for 16+ Supported Living make an introduction?
5. How do we ensure we are receiving referrals for 16+ Supported Living?
6. On average how many referrals were made to Supported Living by the local authority within the boroughs administrative area for tax year April 1st 2024 to March 31st 2025
7. What is the average cost per week to place a young person in a Supported Living placement?
8. Can we please have an email address / contact details to the appropriate team / person in your authority that deals with 16+ Services.

Could you please provide me with the following information, preferably in digital format.
1. Does the local authority currently use Supported Living, 16+ services?
Answer: Yes.
2. If so, how many are currently registered within your borough?
Answer: 4.
3. Is there a recommended service provider for Supported Living list?
Answer: The Service Area have advised Herefordshire are part of the West Midlands Framework.
4. How would a new provider for 16+ Supported Living make an introduction?
Answer: The Service Area have advised to contact our Home Finding Team / Commissioning Team.
5. How do we ensure we are receiving referrals for 16+ Supported Living?
Answer: The Service Area have advised Herefordshire use the West Midlands Framework Portal and email list.
6. On average how many referrals were made to Supported Living by the local authority within the boroughs administrative area for tax year April 1st 2024 to March 31st 2025
Answer: The Service Area have advised 40 referrals were made for tax year 1st April 2024 to 31st March 2025.
7. What is the average cost per week to place a young person in a Supported Living placement?
Answer: The Service Area have advised the average cost per week is £1,282.
