1. Please publish or provide links to the Herefordshire council Policy for informing corporate parents, parents, next of kin, staff of the death of a care leaver under 25 or a vulnerable adult?

2. Is the policy fulfiled by the multi agency service hub? or an assigned staff member?

3. Is a risk assessment made regarding those especially close to the deceased? Mental anguish and adverse reactions.

4. Is there a duty for council staff to inform parents, corporate parents, staff of the death of a care leaver under 25 or a vulnerable adult? 5. If such a policy exists when was it first used?

2. Is the policy fulfiled by the multi agency service hub? or an assigned staff member?

A: Please see policy for these details.

3. Is a risk assessment made regarding those especially close to the deceased? Mental anguish and adverse reactions.

A: Support is discussed with significant others. Plans for support will vary for individuals depending upon individual circumstances, needs and the responsibilities of the organisation to the individual.

4. Is there a duty for council staff to inform parents, corporate parents, staff of the death of a care leaver under 25 or a vulnerable adult?

A: Please see policy for these details. There is no formal policy in place for vulnerable adults specifically. Such circumstances would be dealt with on a case by case basis.

5. If such a policy exists when was it first used?

A: 2013
