A: The Service area have advised that Herefordshire Council do not commission any direct support service specifically for dementia, however they have advised the following:
Herefordshire Council commission 17 x home domiciliary care providers on 2 frameworks (contract extended to 30 Apr 2027) and 85 residential / nursing care homes on a Dynamic Purchasing System (contract extension in the pipeline to 23 Feb 2029), which support our community residents with dementia, but which are not specifically dementia support services.
Herefordshire Council also currently commission Shaw Healthcare Herefordshire to deliver some daycentre provision eg: Leadon Bank Daycentre (again not specifically as a dementia support service) but one which supports people and families / carers impacted by dementia. Please note the daycentre provision is subject to current contract review.
As an active partner, Herefordshire Council’s Ageing Well team regularly attends the below partnership meetings, and participated in an initiative last year to update the Herefordshire Dementia Roadmap Dementia Roadmap - Listings as part of the Herefordshire Dementia Partnership.
• Dementia Programme Board
• Herefordshire Dementia Partnership
• Living Well (Herefordshire and Worcestershire)
Herefordshire Council also fund a number of Local Variation Agreements with individual care homes to provide day care support to individuals where dementia would also certainly be assessed for.
