  4. For the roads Moreland Avenue, Audley Crescent and Courtnay Rise, Hereford, can you please provide the following information regarding PCNs for the past 5 full years (2018-2022) plus 2023 up until the date the information is gathered (please specify).

    1) The amount of PCNs (Penalty Charge Notices) issued to vehicles parked on these roads, per year.

    2) The alleged contravention that occurred to issue each PCN

    3) The road name, if able

    4) The outcome for each PCN (i.e. paid at discounted rate, paid at full rate, cancelled, cancelled following appeal, cancelled following tribunal, upheld following tribunal, etc).

  5. In relation to the Site History Report for B4214:

    1. What training is needed to carry out this task?

    2. How many workforce are needed to carry out this function?

    3. What speed the driver is doing in order to be able to observe all roads in detail?

    4. The pothole in question has been identified in a picture from google maps date November 2022 (please find attached) and yet I cannot find it identified on the site history report as being defective?

    2. A name, email, and postal address of the individual ultimately responsible for assessing appeals and reviewing how safeguarding cases have been handled by local authority personnel and agencies.

    3. The total number of appeals submitted to Herefordshire Council, related to LADO referrals, over the last five years, and the percentage of appeals that were successful. If you do not have an appeals process, could you please provide:

  9. I am researching the varying provision of community equipment services ("CES") throughout the UK.

    A community equipment service provides daily living aids such as crutches, commodes, profiling beds, etc. This service might be known under a different name locally, such as a joint equipment store or an equipment loan service. The service may also provide other equipment in addition to daily living aids.
